Thank you for choosing Sonshine! We are happy to have you. Below is a list of things you will need for enrollment and the first day of class which is always the day after Labor Day. We follow USD 458's schedule for weather days, Christmas Break, and Spring Break. Announcements are posted on the entrance door, on our Sonshine page, and on our website home page. Lisa has an "open door" policy. Call anytime if you have a question.

Parent fills out all four pages. Be sure all spaces are filled out . If something doesn't apply put N/A in that space.
Parent fills out this form and it must be notarized. You can find a notary at the library or your bank.
Items to bring each time child attends:
Book bag with a complete change of clothes including underwear and socks remember when seasons change or growth occurs to switch out appropriate clothing
You are NOT responsible for bringing school supplies or snacks.
Parent fills out all pages except the physician page which must be signed by a physician. The child's shot record can be copied and attached.
